The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on 07/23/2019 regarding physical plant. The inspection was initiated on 08/08/2019 and completed on 08/08/2019. On 08/08/2019, the inspector was on site from 9:30am to 10:20am. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, there were violations found during the inspection regarding staff records. See the violation notice for more details. Standard #: 22VAC40-185-160-B Complaint related: No Description: Based on observation, the center did not ensure that one of three staff records had documentation of a negative tuberculosis screening signed by a physician, the physician's designee or an official of the local health department. Evidence: The record of staff #3 (start date: 5/1/19) did not have documentation of a tuberculosis screening signed by a physician, physician's designee or an official of the local health department. Plan of Correction: I will send [staff #3] to get it done.
